Name Change-O-Chart 2000

There is this chart from a book called Captain Underpants and the Perilous Plot of Professor Poopypants that can create funny names. There are three parts, and it goes like this:

First chart: Use the first letter of your first name to determine your new first name.

Second chart: Use the first letter of your last name to determine the first half of your new last name.

Third chart: Use the last letter of your last name to determine the second half of your new last name.

For example, my new silly name is Booger Gerbil-Head because L is the first letter of my first name, T is the first letter of my last name, and A is the last letter of my last name.
